Social Work Services
Expert psychosocial support, advocacy, and capacity building - delivered with integrity.
NDIS Capacity Building Supports
At Holistic Care Therapy, our Social Work services support NDIS participants who experience psychosocial, environmental, and systemic barriers that impact their capacity to participate in everyday life, relationships, and community. Our Social Workers are senior clinical practitioners who bring strong advocacy skills, extensive experience working within complex systems, and a commitment to collaborative, strengths‑based practice. Services are practical, trauma‑informed, and person‑centred. All Social Work services are delivered in line with NDIS Practice Standards, the NDIS Code of Conduct, and professional ethical frameworks.
Who Social Work Is For?
Supports are tailored to each participant’s goals, strengths, lived experience, and environment. Social Work may support NDIS participants who:
Experience psychosocial disability or complex mental health needs
Face barriers navigating housing, health, education, justice, or child protection systems
Require advocacy, coordination, and system navigation
Are experiencing transitions, instability, or periods of increased vulnerability
Benefit from structured psychosocial assessment, reporting, and capacity building
What Our Social Work Services Include?
- Psychosocial assessments with a functional and environmental lens
- NDIS reports and supporting documentation grounded in psychosocial impact
- Ongoing psychosocial support and capacity building
- Strong stakeholder engagement and team‑based collaboration
- Advocacy and representation across systems
- Systems navigation (NDIS, housing, health, education, justice, OOHC)
- Family and carer support, education, and guidance
- Training and capacity building for families, carers, and support teams
- Support during life transitions and periods of change
- Risk‑informed and trauma‑aware intervention
- Collaboration with multidisciplinary and community services
Our assessments, reports, and interventions clearly articulate how psychosocial factors impact functional capacity, participation, and daily living, supporting evidence‑based planning and funding decisions.
